Akismet has been fairly good recently.  Haven't had any problems recently with it.  We did remove it from all of our client's sites though and now run our own solution. (It's kind of fun watching an IP address get tagged off of one site and watch all of it's connections drop at the firewall when it starts going nuts.) I know some of the places I help out at removed it as well.</description>
